All required laboratory tests are to be done at an outside laboratory and the fees are not included.

1st follow up consultation.............................................. $250.*

2nd and further follow up consultations are each ........$125.*

*Payment is to be made in full in cash or by check prior to the consultation

*You will have to check with your health insurance company if obesity is a covered diagnosis. You should find out how much they will reimburse you and how many visits they will cover. We will submit forms for you to you insurance carrier.
